CCYPCG home / The blue card system / What form do I need? / Blue card forms for new applicants and renewals
Following a review of the Blue Card application process, the Commission has made changes to streamline application forms. Consequently, there are no longer separate forms for first time applicants and renewals.
Please click on the relevant application form link below:
Some employers have pre-filled forms. Speak to your employer about whether you should access your form from the Commission’s website or whether they are able to provide you with a pre-filled form.
Government employees have special forms and should refer to their HR department.

Business operators, volunteers and trainee students must submit their renewal application at least 30 days before their blue card expires to continue working while their renewal application is being processed, unless their blue card has been suspended or cancelled.
Paid employees must submit their renewal application before their blue card expires to continue working while their renewal application is being processed.
The Commission will send card holders an invitation to renew their blue card 16 weeks before their card expires. Please notify the Commission if you change your contact details.
Please note, exemption cards do not have an expiry date and remain valid as long as the card holder remains a registered teacher or police officer in Queensland.
