Blue card holders, applicants and those issued with negative
notices must advise the Commissioner if their name or contact
details change.
The Commissioner must be notified of any changes within 14 days otherwise a penalty may apply.
To notify the Commission of a change, complete the appropriate form in full and forward it to the Commission.

Change of name
To notify the Commissioner of a change to your name, you should complete a change of name form.
You should also attach a certified copy of the document evidencing your name change (eg Marriage Certificate or Change of Name Certificate). If you do not have a document which evidences your name change you should complete the following statutory declaration and attach it to your change of name form. You will need your Statutory Declaration witnessed by a Justice of the Peace (JP). The Department of Justice and Attorney-General website has a list of JPs in Queensland.
Change to address or phone number
To notify the Commissioner of a change to your address or phone number, you should complete a change to contact details form.
